In Financial Year 2005 WCI acquired the resources to hire a Full-time Health Care Coordinator.  The Full-time Health Care Coordinator supports Individuals and staff by providing systematic and random reviews of the Individual's Health Care Record and assisting with maintaining compliance with DMR's Medication Administration Program (MAP).  Because the health care concerns of many Individuals are both diverse and complex and span across many health care specialities, WCI utilizes a Registered Nurse who has an understanding of the many health related issues that arise.  The Health Care Coordinator provides written strategies and training associated with preventive health care.  Focusing on these strategies allows a home to run smoother and creates more time for Individuals to be involved with their communities.

The Health Care Coordinator provides 24 hour a day, 7 days a week on-call coverage that is a requirement of the DPH MAP Regulations.  The Coordinator provides short-term medical case management with those Individuals whose care is complicated and/or acute and professional oversight with most of the other Individuals serviced by WCI.  The Health Coordinator collaborates with the Clinical Team, key support staff and Management to promote the best outcomes with those Individuals and homes with neurological/psychological issues as well.

Developing systems to maximize support provided by 3rd party services is important in maintaining Individuals in their homes.  The Coordinator facilitates, communicates and coordinates hospital admissions and discharge.  This need could develop from hospital discharge planning, treatment of new situations or through systematic or random review of the Health Care Record of the Individual.

The Coordinator also does intake assessments on new referrals to the agency.  The Health Care Coordinator assists or provides trainings in all areas related to health, medical conditions and medication regulations.  Not only can the Coordinator improve communication with the various Health Care Providers (HCPS), she can provide trainings with support staff on how to communicate more effectively with them.
